What is an ACORD 25?
The ACORD 25 — the Certificate of Liability Insurance — is the one-page form contractors use to prove they carry insurance. It summarizes the producer (the insurance agency), the insured, each insurer, and the active policies (general liability, automobile liability, umbrella/excess, and workers' compensation) along with their limits and effective and expiration dates. General contractors and owners ask their subcontractors for one before work starts.
How to fill out an ACORD 25
Enter your agency and the named insured, add a row for each coverage with the policy number, limits, and dates, then list the certificate holder (often the GC requesting it) and a short description of operations. Mark the additional insured, waiver of subrogation, and primary & non-contributory boxes where your policy provides them — these are the endorsements GCs check most. Then download the PDF.
Important: a certificate of insurance is informational only and confers no coverage. This tool produces a clean, free look-alike certificate — it is not the official ACORD® form and is not endorsed by ACORD. Use it to produce or draft a certificate; your insurer issues the binding document.

FAQ
Is this the official ACORD 25 form?
No. This is a free, clean look-alike Certificate of Liability Insurance generated by StrikeDocs — not the official ACORD® form and not endorsed by ACORD. It lays out the same standard fields so you can produce a certificate quickly.
Does filling this out give me insurance coverage?
No. A certificate of insurance is informational only and confers no coverage or rights on the holder. Only your insurer and policy provide coverage. The certificate simply summarizes policies that already exist.
Is it free?
Yes — completely free, no signup and no card required. You fill the fields and download the PDF.
How do I fill out an ACORD 25?
Enter the producer (agency), the insured, each insurer, and a row per coverage (general liability, auto, umbrella, workers' comp) with policy number, limits, and effective/expiration dates. Add the certificate holder and a description of operations, then download.
Can I check a certificate against a general contractor's requirements?
Yes — after you create one, StrikeDocs can check the limits and endorsements against a specific GC's insurance requirements and monitor the policy's expiration. That's the paid product; the filler is free.
